New Client Forms

New clients must have forms filled out before the Initial Appointment.  You can either fill out the paperwork in the office before the appointment (please allow 10 minutes-so get there a little early), or you can download the paperwork needed and bring it in completed.  All pages are needed.  Please use the appropriate information sheet, either Adult or Child, depending on the age of the patient.
